Quick answers: HCG doses in units
With a 10,000 IU vial mixed with 10 mL of bacteriostatic water (1,000 IU/mL), on a U-100 insulin syringe:
- How many units is 250 IU of HCG?
- 25 units (0.25 mL)
- How many units is 500 IU of HCG?
- 50 units (0.5 mL)
- How many units is 1,000 IU of HCG?
- 100 units (1 mL)
- How many units is 1,500 IU of HCG?
- 150 units (1.5 mL), more than one 1 mL syringe
- How many units is 2,000 IU of HCG?
- 200 units (2 mL), more than one 1 mL syringe

How much BAC water for 5,000 and 10,000 IU HCG
The labels set the volumes. Novarel 5,000 and 10,000 unit vials come with 10 mL of bacteriostatic water: 10 mL in a 5,000 IU vial gives 500 IU/mL, and 10 mL in a 10,000 IU vial gives 1,000 IU/mL. The Pregnyl label says to add 1 to 10 mL of its supplied solvent, so its concentration depends on the volume you choose.
Many people mix a 5,000 IU vial with 5 mL to get 1,000 IU/mL, where every 10 units on a U-100 syringe is 100 IU. Less water makes each draw smaller; more water spreads it over more marks. Whatever you choose, write the IU per mL on the vial so later doses use the same number.
- 5,000 IU + 5 mL = 1,000 IU/mL → 500 IU = 0.5 mL = 50 units
- 5,000 IU + 10 mL = 500 IU/mL → 500 IU = 1 mL = 100 units
- 10,000 IU + 10 mL = 1,000 IU/mL → 250 IU = 25 units

Educational reference only — not a dose recommendation. Units assume a U-100 insulin syringe (100 units = 1 mL on U-100); “per vial” is how many of that dose the vial holds, before the small dead-space loss on each draw. Confirm against your own vial label, diluent, and clinician or pharmacy instructions.
How to mix an HCG vial
Use the diluent that came with the vial: Novarel is supplied with bacteriostatic water and Pregnyl with its own solvent, both containing benzyl alcohol as a preservative. Wipe both stoppers with alcohol, draw the diluent, and let it run slowly down the inside wall of the vial. Swirl or roll gently until the powder dissolves — do not shake it — then write the date and the IU per mL on the vial.
- Label the vial with the date and IU per mL
- Refrigerate after mixing; do not freeze
- Recalculate if you change the water volume on the next vial
What the HCG labels and guidelines say about doses
The Novarel and Pregnyl labels list hCG for hypogonadotropic hypogonadism in men, with regimens such as 500 to 1,000 units three times a week for three weeks and then twice a week for three weeks, or 4,000 units three times a week for six to nine months followed by 2,000 units three times a week for three more months. The same labels state that hCG has not been shown to help with weight loss.
The American Urological Association's testosterone guideline lists hCG as an option for men with testosterone deficiency who want to preserve fertility, and a small study used 500 IU every other day alongside testosterone therapy. This page does not recommend a dose: use the one your prescriber gives you and the calculator to turn it into units.
How long HCG lasts after mixing
Each brand's label gives its own limit. Novarel says to refrigerate the mixed vial at 2–8 °C and use it within 30 days; Pregnyl says it is stable for 60 days refrigerated. Do not freeze either one. For any other product, follow the date on its label or from your pharmacy.
Looking for a pregnancy hCG calculator?
Pregnancy hCG calculators interpret blood test results, such as how fast hCG levels double in early pregnancy. This page does something different: it works out how much liquid to draw from an hCG injection vial. If you are tracking pregnancy test results, ask the clinic that ordered the tests.
