The semaglutide weight-management label (Wegovy) starts at 0.25 mg once a week for 4 weeks, then steps up every 4 weeks through 0.5, 1 and 1.7 mg to a 2.4 mg maintenance dose. From a 5 mg vial mixed with 2 mL of bacteriostatic water (2.5 mg/mL), 0.25 mg is 10 units and 0.5 mg is 20 units on a U-100 syringe.

  1. 1Find the concentration

    5 mg × 1,000 ÷ 2 mL
    = 2,500 mcg/mL

  2. 2Work out the volume

    250 mcg ÷ 2,500 mcg/mL
    = 0.10 mL

  3. 3Convert to units

    0.10 mL × 100
    = 10 units

On a U-100 insulin syringe, 100 units equals 1 mL, so each unit is 0.01 mL. The 0.3 mL (30 unit) and 0.5 mL (50 unit) syringes use the same scale, just shorter, which makes small doses easier to read.

How to read the semaglutide dosage chart

Each row is one step of the Wegovy titration and each column is a common compounded vial set-up. The cell shows the units to draw on a U-100 insulin syringe, the dose in mL, and how many of those doses the vial holds. Semaglutide doses are small, so the units are small too — which is why the FDA has warned about dosing errors with compounded vials, where people confuse mg, mL and units.

Always convert the milligram dose for your own vial. 0.25 mg is 10 units from a 5 mg vial in 2 mL, but 5 units from a 10 mg vial in 2 mL and 12.5 units from a 2 mg vial in 1 mL.

  • 5 mg + 2 mL = 2.5 mg/mL → 0.25 mg = 0.1 mL = 10 units
  • 10 mg + 2 mL = 5 mg/mL → 0.25 mg = 0.05 mL = 5 units
  • 2 mg + 1 mL = 2 mg/mL → 0.25 mg = 0.125 mL = 12.5 units

The Ozempic (type 2 diabetes) schedule in units

Ozempic uses a shorter ladder: 0.25 mg for 4 weeks, then 0.5 mg, with optional increases to 1 mg and then 2 mg after at least 4 weeks on each dose if more glucose control is needed. The maximum is 2 mg a week, below Wegovy's 2.4 mg.

How long a semaglutide vial lasts at each step

At the starting dose a vial goes a long way: a 5 mg vial holds twenty 0.25 mg doses, which is far longer than most reconstituted beyond-use dates. Near maintenance the same vial holds only two 2.4 mg doses. Many people choose a smaller vial for the first steps and a larger one later.

  • 2 mg vial: 8 doses at 0.25 mg, 4 at 0.5 mg, 2 at 1 mg
  • 5 mg vial: 20 doses at 0.25 mg, 5 at 1 mg, 2 at 2.4 mg
  • 10 mg vial: 40 doses at 0.25 mg, 10 at 1 mg, 4 at 2.4 mg

Some prescribers hold a step for longer or use doses between the label steps, often described as microdosing, to manage side effects.
